Each team must be eligible under the State Playoff rules.  If your teams needs an exception, click on the registration below.  MAHA does not accept the old Excel exception forms.  This is the only method for requesting an exception.

If your team or player requires more than one exception, fill the form out for each type and submit.

1. Check in the amount of TBD payable to MAHA D2 (N/A for AAA or Girls).  This needs to come from the Association- one check for all the teams participating.
2. Completed Credentialing form (Front & Back).  Back page with player game counts to be supplied by your local register just before the credentialing meeting so that it will be up-to-date.
3. Team Roster (Certified copy)
4. Copy of government issued birth certificate, or, if birth certificate is not available, copy of a passport ID Page for each rostered player.  Non-US citizen must provide proof of legal residency and proof of release and transfer from home country Ice Hockey Federation.  Birth certificates are not needed if roster is stamped with the birth certificate verification from the registrar.  
5. Game log showing all games played (League, Scrimmage & Tournament).
6. Score sheets for all games played.

Note:  "Student Coaches must sign a hard copy of the Coaching Ethics form which must be turned in to the District Registrar.  All other coaches, the Ethics form is now part of your Background Check.
